Announcements

We still have seasonal flu vaccine in nasal and injectable forms. We received a small amount of injectable H1N1 vaccine for ages 6 months through adult.  Please note that the vaccine for children 6-35 months that we currently have is not preservative free.  We also have H1N1 FluMist for healthy children ages 2 and older. Having a new baby? Call our office to schedule a prenatal visit to meet one of our doctors and tour our state-of the-art facilities. The time is 5:30pm until all questions have been answered. Please call to schedule a visit. We look forward to meeting you. When switching from another practice to White Oak Pediatrics you will need to have your child's records transferred to us. A medical release form is available to download. Having the medical records before your child's first visit is strongly suggested, however this is not a requirement. White Oak Pediatrics is now offering the 3 shot series of HPV vaccine. HPV helps protect young women from some types of Cervical Cancer. Please call to schedule an appointment. New Patients: Please download both the Registration Form and Health History Form.This will save time on your first visit if the forms are completed upon arrival. Established patients: If you have any insurance, address or employment changes please download a new Registration form. The completed form can be faxed to 919-571-9314. Thank you.
